Pre-Kindergarden Registration

Dear Hasbrouck Heights Residents & Board of Education Employees,

Pre-K 4 registration for the 2009-2010 school year will be held on Friday, March 20, 2009, at the Eglow Media Center at the High School from 6:30-8:30 p.m.  Applications will be accepted on a first come/first serve basis.  You will not be registered unless you have all required documentation.  Class size is 20 students.  If there are more than 20 students registered, they will then be put on a waiting list.

 In order to complete the registration process, download, complete and bring the following documentation with you to registration along with a check for $400.00 which will be applied to the tuition of $4000.00 for the year. These documents can be obtained from the HHBOE website- www.hhschools.org.

  • A completed District Registration Form
  • A Physical examination Form It must be dated and signed by the medical provider
    • The office must stamp the form giving its name, address and phone number.
  • An Immunization Form  It must include the signature of the physician
  • A Dental Exam Report Form It must include the date, dentist’s name and signature.
  • A Health History Form
  • Health Screening Form
  • An Authorization for Sharing Information Form
  • Bring a copy and the original of the following documents:
    • Proof of Residency: Property tax bill, deed, contracts of sale, leases, mortgages, signed letter from landlord etc…)
    • 4 of the Following Proof of ‘Attachment to Address’:  Voter registrations, licenses, permits, financial account information, utility bills, delivery receipts, and other evidence of personal attachment to the given address)
      • Photo ID:  Drivers license, county residence, employee ID, passport
        • Student Information:
          • Birth Certificate
          • Immunization Record
          • Proof of Physical Exam
          • Proof of Dental Exam
